C19 AstraZeneca Vaccine Eligibility Criteria

Please ensure you, or the person you are booking for, are eligible to receive the AstraZeneca vaccine before booking this appointment.

We are currently able to offer the AstraZeneca vaccine to the following groups:

• People aged 60 years of age and over
• People aged 18 to 59 years of age - GPs will assess suitability and discuss potential risks with the patient prior to reaching a mutual decision if they should receive the vaccine
• As a booster dose to people aged 18 years and above AND
• who had their second COVID-19 vaccine dose more than 5 months ago AND
• who had a first course of AstraZeneca without any complications OR has a contraindication to a mRNA vaccine (Pfizer OR Moderna) such as anaphylaxis after a previous dose of an mRNA vaccine AND

Do not proceed if you are under the age of 18 years.

Please bring evidence of any previous COVID-19 vaccinations given by a different provider

Clinical suitability to receive a COVID-19 vaccine will be determined by the GP.
