Medical Certificate Eligibility

This service is only available for existing patients. Requests for medical certificates may not be available for all patients and may also require additional information from your doctor.

In the event we are unable to service this request, a refund will be issued and we advise you to make an appointment at the clinic.

Each medical certificate request is assessed by your doctor who will approve or decline the request on clinical grounds. If the request is declined, you will be advised to make an appointment in person.
